Health Prevention Assistant

The Health Prevention Assistant project in the SKCH LAG area, reg. no. CZ.03.2.65/0.0/0.0/16_047/0009874, was financed from ESF funds through the Operational Program Employment.

The aim of the project was to mitigate the impact of social exclusion on socially excluded individuals and those at risk of social exclusion in the Chrastecko, Skutečsko, and Košumbersko LAG areas, specifically targeting the prevention of inequality in healthcare. Services were provided in the field.

Who was the project intended for?

• Individuals over 18 years old who are socially excluded or at risk of exclusion and living in the Skutečsko, Košumbersko, and Chrastecko LAG areas,
• Individuals caring for a close person,
• Individuals at risk due to specific health risks (with reduced social or economic status),
• Individuals in unfavorable life situations with limited access to information, services, and institutions in the field of health prevention and healthcare.

What did the project offer?

• Provision of information on health, healthcare system, and related services,
• Assistance services in dealing with health services and institutions,
• Assistance in renewing and processing health-related documents (insurance card, disability card, health status certificate, etc.),
• Support in dealing with health insurance companies,
• Accompaniment to healthcare and related service facilities,
• Basic legal advice in the health field,
• Educational support for a healthy lifestyle.

• Provision of services for FREE,
• Discretion, the option to use services anonymously,
• Support for independent resolution of needs,
• Individual approach,
• Respect for the user's free will.

The Health Prevention Assistant project was implemented from 1st November 2018 to 31st October 2020.
